IMPORTANT NOTE: The maximum allowable gross income will be adjusted annually by the consumer price index as reported by the United States Government. If this income threshold is surpassed this assessment freeze will be rendered invalid. The Special Assessment application, if granted, is valid only for as long as (a.) that owner, who is sixty-five years of age or older, or that owner’s surviving spouse who is fifty-five years of age or older, or who has a minor child(ren), remain the owner and occupant of the property, (b.) the value of the property does not increase by more than 25% because of construction or reconstruction. In the event the property is sold, the Special Assessment Lever shall terminate on the last day of December in the year the property was sold. I have read the above information and certify that the information regarding my personal qualifications is true and correct. I understand that it is a criminal offense to make any false statements for the purpose of procuring a Special Assessment Level. I am also applying for the Special Assessment Level in accordance with Art. 7:18(G) (1) of the Louisiana Constitution. I am 65 years of age or older and have a gross income of no more than the adjusted gross income listed above.
