Proof of Disability

You must provide a copy of your disability letter from the Social Security Office. You must be 100% disabled to qualify.

Adjusted Gross Income for the Year Prior to Application

You must provide a copy of the prior year’s tax return. Upload your file below.

IMPORTANT NOTE:

This application may be applied to the homesteaded property of the applicant if they meet all requirements stated below. If granted, the special assessment level will freeze the assessed value of the property as valued upon granting the application. The freeze will remain on the property as long as the applicant is eligible. The property owner will still have to pay any additional taxes that are a result of a millage increase.

REQUIREMENTS:

A. Applicant must be permanently totally disabled as determined by a final non-appealable judgment of a court or is certified by a state or federal administrative agency charged with the responsibility for making determinations on that matter.

B. Adjusted Gross Income must not exceed $102,700 a year. If property has more than one owner, you must combine incomes if you file separately.

The maximum allowable gross income will be adjusted annually by the consumer price index as reported by the United States Government. If this income threshold is surpassed this assessment freeze will be rendered invalid.
